Iran has the second highest rate of gender reassignment surgeries in the world, behind Thailand. Iran’s government subsidizes surgeries and medical interventions that allow people to live as the other gender. This is usually done because the person is homosexual, which is punishable by death and beatings in Iran. However, if one changes their gender, they are able to love who they want, but at great personal cost of infertility, and painful surgeries that may result in limited or diminished sexual function.
